Instructions for Filling Out the ID Card Form

Please Follow These Instructions to Submit Your ID Card Form

Dear Students,

You are required to fill out the ID Card Form without delay.
🔴 All fields are mandatory. No incomplete submissions will be accepted.

Forms submitted after this date will NOT be accepted under any circumstances. No extensions will be granted.

✅ Information Required:

  • Programme enrolled

  • Admission Form No (from eSamarth)

  • Complete Roll Number (e.g., 2501001, 2502010, 2503030)

  • Full Name (as per HSSC marksheet)

  • Date of Birth: dd/mm/yyyy format

  • Blood Group : Select the list from the drop down(eg. A+, B-)

  • Permanent Address: Mention key details to identify the address clearly without unnecessary info (Eg. House/Building No., Street/Locality, City/Town, State, PIN Code)

  • Contact Number: Student’s personal phone number.
  • Emergency Contact Number: Phone number of a parent, close relative, or guardian.


📸 Photograph Instructions (Strictly Followed)

  • The photo must be taken against a plain white background.

  • Ensure no shadows, no hats, no tinted/dark glasses, and nothing obstructing the face.

  • Pose should be front-facing, full face visible (shoulders up), eyes open, and looking at the camera.

  • No filters, no beauty enhancements, and no blurry images.

  • Image format: .jpeg/jpg only.

  • Rename your photo file to your assigned Roll Number before uploading.


Unacceptable Photos (These will be rejected):

  • Dark or unclear images

  • Face is not fully visible

  • Background includes scenery or patterns

  • Funny faces, tilted angles, or silly expressions

  • Poorly cropped or poorly framed photos


Failure to follow these instructions will result in your ID card not being processed.
